loss of power

Hi there...newbee here so ...my 406 99 ls td has a problem...comeing back from a day in the mountains the car had a very slight miss another five minutes on the road the engine management light came on( never seen this light before) then she dropped power with a lot of white smoke(heavy diesel smell)...managed to get home....thinking it was dirty diesel I changed fuel filter ...car starts and will idle forever no problem...but when I try to drive it ..back comes the white smoke and loss of power..checked hoses,leads cant see anything amiss...she has done 164000mls...any ideas?
